Physical Therapist II Home Health

As our Physical Therapist, you will help Hospice/Home Healthcare patients perform all phases of physical therapy evaluation, treatment planning, treatment, and documentation so they can achieve their rehabilitative goals and improve their quality of life in a home setting.

Every day you will evaluate patients, participate in treatment program planning, and administer physical therapy modalities, procedures, and tests.

You will be expected to perform patient initial and re-evaluations, administer treatment plans using accepted therapy procedures, document daily on appropriate forms, communicate regularly with the medical team, maintain standards of conduct, and comply with all organizational policies.

To be successful in this role, you must have at least two years of professional experience in rehabilitation, maintain compliance with professional standards of conduct, and effectively communicate patient progress to the medical team.

• Perform patient initial and re-evaluations, establishing and implementing effective treatment plans.
• Administer treatment plans using accepted therapy procedures, overseeing therapist extenders (aides) in their implementation.
• Document daily on appropriate forms using clear and concise data, and communicate patient status/progress effectively.
• Communicate regularly with other members of the medical team regarding goals, progress, and rehabilitative potential.
• Maintain the standards of conduct as promulgated by the American Physical Therapy Association and adhere to the professional code of ethics.
• Comply with safety, infection control, Dignity Health Integrity Program, and other organizational policies.

Required

• Two (2) Years Of Professional Experience In Rehabilitation.
• Active Physical Therapist (PT) license in the State of California.
• Basic Life Support – Cardiopulmonary Resuscitation (BLS-CPR) through American Heart Association.
• Driver's License: California.
• Auto Insurance.
